Privacy Practices Notice

This notice describes how your medical information may be used and disclosed and how you can get access to this information.  Please read it carefully.  The privacy of your medical information is important to us.   

What Should I Bring?

Contact Information
You will be asked to provide your Social Security number, address and telephone information.  

Insurance Information
If you have an insurance card, bring it.  In order to bill your insurance carrier(s) for services rendered, we will need the name of the carrier (company), their complete mailing address, your policy's group number and your personal identification number.  If your insurance carrier requires a referral form, please bring it with you to your first appointment.  If Medicare or Medicaid covers you, please bring the appropriate card as well.  You may also be asked to pay for services not covered by insurance.  

Medical History
If your visit is for a second opinion or on a physician's referral, you may need to bring medical records, x-rays, results from lab tests, pathology slides or other materials.  Your physician may have sent these to us already — be sure to ask your doctor before your visit here.  

Advance Directive
If you have an Advance Directive - a living will and/or durable power of attorney for health care - please bring it with you.  To learn more about advance directives and to access downloadable forms, click here.      

Same-Day/Outpatient Surgery

What Should I Bring If I Need To Stay Longer?

* Personal toiletries, bathrobe, slippers or socks.  
* Any medications you usually take.  For a printable list you can fill out and bring with you, click here.  
* Your glasses, dentures, hearing aid, etc.  
* Any written instructions your physician may have provided.  
* A list of your current prescriptions is also helpful.  Be sure to write your name on the containers and inform your nurse.   

What Should I Leave At Home?

Jewelry & Other Valuables
Peace River Regional Medical Center has a safe for valuables in emergency situations only.  We ask that you leave all valuables at home or with a family member when you come for outpatient procedures or inpatient care.  We are not responsible for personal items that are lost or stolen on our premises.  

Money and Credit Cards
A lot of money or extra credit cards are not necessary.  (You may, however, want a few dollars to buy a magazine or book from the Gift Shop).  

Electrical Appliances
Hair dryer, electric shaver, radio or other electrical appliances.   

Patient Education and Television Channels

Peace River Regional Medical Center utilizes the latest in UL approved Hospital Grade LCD television technology to provide a comfortable and patient centric atmosphere during your — and your loved ones — stay.  Patients can enjoy the digital quality of broadcast and top-rated cable  programming — plus digital on-demand patient education movies — free of charge.

Continuous Ambient Relaxation Environment (CARE): Tune into Channel 45 for beautiful nature images and instrumental music that has been composed and produced specifically to support a healing environment.

Video Education on Demand: Tune into Channels 47, 48, and 49 for topics related to heart health, diabetes, respiratory disease, joint disease, pain management and more.  Follow the directions on the screen or ask your caregiver for assistance.  For a complete TV listing, please refer to your Patient Information booklet.
