The Congregational Health Network (CHN) is a unique opportunity for both hospital and congregations to partner in providing optimal health, healing and well-being for the people in our community.

Many congregations already offer outreach by providing both spiritual care and support to our patients.  The CHN is our outreach to congregations to provide training and to equip them to provide health advocacy for their members.

What is an Advocate?

An “advocate” is someone who helps, defends, counsels and comforts.  In our often complicated medical systems, we need advocates to help bridge the gap between hospital and home.  This is the primary goal of the CHN.

How do I find out about this program?

A community relations registered nurse meets with local clergy and facilitates the training of a core group of health advocates in their congregation, providing access to specific health educational needs.

Is there a fee for this service?

This service is without cost to the congregation.

The training session is 1 1/2 hours and is interactive. Once a date and time is set up, our nurse will come to the congregation to give the training.
